BVCHEA teams are required by our league (TAIAO) to have a medical release form on file for all players. Sports physicals can be done by any medical professional. A copy of the UIL form can be downloaded from BVCHEA.org downloads page. Similar forms will be accepted. The form is good for two years from the date of the exam.
You can upload a scan or a photo of the completed form to your Teamsideline account, under the family member (player) you are submitting it for.
This process can also be followed for storing transfer forms and proof of grade level (for specific cases).
Please follow the steps below, INCLUDING the last one - clicking SAVE after you've uploaded the file.

1. From a WEB BROWSER (not the Teamsideline app), go to BVCHEA.org and sign into your account.
2. Click MEMBERS.
3. Click the EDIT icon (black pencil icon) to the right of the player name. Do not click the player name. Enter the date of the exam and click "select" to upload the document.
4. DO NOT FORGET to click the SAVE button at the bottom of the page!
