BVCHEA participants are covered under K and K Insurance.
Below is the process for handling insurance claims for injuries:
1.) Complete PART I of the participant accident claim here: https://www.kandkinsurance.com/claims-insurance/participant-accident-claim
2.) After completing PART I, have the BVCHEA Board President or Athletic Director sign the form
3.) Complete PART II of the form.
4.) Submit PARTs I & II of the form to K&K insurance per their instructions on the form
5.) Determine how you will utilize K&K insurance, you have two options:
OPTION 1: Place K&K insurance on file with your medical providers as secondary insurance. If you do not have medical insurance, you would place K&K on file as your primary insurance) Once the medical provider runs their invoices through your primary insurance they will automatically run any remaining balance due through K&K. This is the easiest and most efficient way to handle your medial claims. However, some medical providers will not accept K&K insurance, thus you will have to complete Option 2 for these situations.
OPTION 2: You will need to pay your medical provider any remining balances due after having been paid through your primary insurance. Once paid you will need to submit the following documents to K&K in order to receive a reimbursement or partial reimbursement of your out-of-pocket payments:
- Copy of the actual invoice from the medical provider
- Copy of your receipt showing payment to the medical provider
- Copy of your explanation of benefits for the procedure(s) listed on the above invoice.
- If any of the three items above are missing, K&K will not be able to complete their claim for the submitted invoice
